Program Terms & Financial Disclosure
Please review the following information carefully before completing your enrollment.
Program Overview
The Weight Optimization Program is a physician-guided medical program focused on weight management and metabolic health.
State Eligibility
Monthly Program Fee: $99 per month
This program includes:
Physician review and oversight
Initial baseline bloodwork included in the program price
Review and interpretation of bloodwork results
Ongoing program access while enrolled
If you have recent bloodwork available, the physician may elect to review and use those results when clinically appropriate. However, baseline bloodwork is included in the program regardless and may still be required at the physician’s discretion.
Medical Eligibility
Enrollment in this program does not guarantee eligibility for prescription
therapy.All treatment decisions are made by a licensed physician after
review of required medical intake forms and, when applicable,
bloodwork.No prescriptions will be issued until medical eligibility is
confirmed.
Medication Costs
Prescription medications are not included in the monthly program
fee.Medication costs are paid separately and directly to the
dispensing pharmacy.Insurance coverage, copays, or cash pricing are determined
by the pharmacy and insurance provider, not by Healthspan
by Design.
Billing & Cancellation Policy
The program is billed monthly at $99.
You may cancel your membership at any time with at least
7 days’ notice prior to your next billing date.Cancellations requested after a billing cycle has begun will
take effect at the next billing period.No refunds are provided for partially used months or services
already rendered.

Weight loss results are not guaranteed.
Program participation does not replace primary care or emergency
medical services.Program fees cover medical oversight and included services
only and do not include medications unless explicitly
